COVID-19 Screening Tool

The safety of our tenants, employees, trades and suppliers is our top priority.

To prevent the spread of COVID-19 in our community please complete our screening tool to ensure your household is not experiencing any symptoms. An AON representative may contact you before anyone enters your apartment to ensure there have been no changes to the information you have provided.

Have you or anyone in your household experienced any cold or flu-like symptoms in the last 14 days (including fever, cough, sore throat, respiratory illness, difficulty breathing)?
Have you or anyone in your household travelled outside of Canada in the last 14 days?
Have you or anyone in your household been in close contact with or cared for someone diagnosed with COVID-19 within the last 14 days?
Have you or anyone in your household been in contact with anyone who has travelled outside of Canada within the last 14 days?

Thank you for taking the time to answer these questions. Our trades and employees will remain the recommended distance away to ensure we are practicing physical distancing. We ask that you and your family remain in another room, such as the bedroom, while the work is being performed. If further communication is required it will be done by telephone or email. The individual(s) doing the work will shout out to you when they are finished.
